Congenital heart disease remains the number one cause of death from a birth defect in the United States. The MGHfC team works with DOCF to address this problem by:

• Providing pediatric/congenital heart care across the left spectrum fetus to adult
• Ensuring all children needing pediatric heart care can receive it, whether that be through providing surgical care for lower complexity cases as well as complex cases in teenagers, or having Boston Children’s handling the more complex for young patients (for example, heart transplants).
• Leading and collaborating in activities that will continually improve patient care regionally and internationally

Freely offering assistance and actively responding to requests for help related to the care of patients with suspected or actual pediatric/congenital heart disease.

$5,000 per surgery is the minimum portion of each surgery that is not covered by insurance reimbursement. The unreimbursed cost for each surgery can be much higher, depending on each patient’s insurance coverage, as well as the severity of each patient’s health situation. DOCF funds unreimbursed surgical costs for 15 pediatric cardiology patients per year.

With timely diagnosis and appropriate treatment, the vast majority of infants and children with congenital heart disease can be saved. Those with more severe heart disease need cardiac surgery or cardiac catheterization for survival. Following these procedures, these children require care in a pediatric intensive care unit setting under the supervision of the team of pediatric heart specialists. The team of dedicated cardiac surgeons, cardiologists, cardiac anesthesiologists, nurses and imaging specialists are trained in treating pediatric heart conditions.

In addition to fully supporting their medical needs, these families also require additional support from child life services, social work and financial services. MGHfC provides a surgical and catheterization based approach to the care of both local and international children with congenital heart disease. These patients typically require lifelong care and return to see the team in the outpatient clinic on a regular basis as they grow into adults.

As part of its support for improving care for pediatric cardiology patients in New England, the David Ortiz Children’s Fund supports the Echocardiography Quality Improvement Project (EchoQIP), a program designed to reduce diagnostic errors among infants and children across multiple pediatric heart centers throughout New England.

Why this program?

Pediatric heart surgery has led to significant reductions in deaths caused by congenital heart disease, however, rapid surgical or medical treatment requires accurate diagnosis of the heart defect. Diagnostic errors or failure to diagnose heart disease places infants and children at risk for permanent injury and death. Diagnostic errors are responsible for a large percentage of hospital complications, cause greater patient harm than medication errors and are a leading cause of permanent organ damage. Diagnostic errors are frequently caused by incomplete or incorrect cardiac imaging by echocardiography. Training and following standard procedures have significant positive impact on error rates.

Thus after the coaching/training component, EchoQIP then measures improvement and diagnostic accuracy and imaging quality of pediatric echocardiograms. In total, this program has provided 12 New England hospitals with training, thereby helping to positively impact between 2,000 and 3,000 pediatric lives.

Pediatric Cardiovascular Program

Pediatric Cardiovascular Program at Cedimat Hospital

In the Dominican Republic, the need for better and more accessible children’s health care is significantly greater than most neighboring developing countries. According to the World Health Organization, congenital health anomalies in the country are twice the average rate of the region, and they are the second most common cause of death behind premature births for those under five years old. Despite recent improvements, the Dominican Republic still has one of the highest infant mortality rates in the region, according to the CIA. Against this challenging backdrop, our program represents a critical source of care and a model for increasing health care access for the country’s most vulnerable population.

To respond to this urgent need, DOCF has partnered with CEDIMAT to make it the only high-quality pediatric cardiac care resource in the country accessible to families who do not have the ability to pay. The hospital’s pediatric cardiovascular unit now has the advanced equipment and highly-trained staff necessary to perform complex surgeries, and they provide vital follow up care that is often not available elsewhere. Through our program, low-income patients can receive:

• Free screenings and diagnoses during annual rural outreach trips
• Common and complex pediatric cardiac surgeries, such as congenital, valve replacement and repair including rheumatic, performed by CEDIMAT’s surgical team
• Highly complex pediatric cardiac surgeries performed with visiting International Mission Teams from across the United States, which also serve to train the CEDIMAT team

More than 80% of the children who come to CEDIMAT and are in need of cardiac surgery require financial assistance. Basic government-provided health insurance only covers up to 60-70% of the cost of the surgery—which costs on average US$15,000 to US$20,000—and the remaining balance is prohibitive for low-income families, who usually earn only $3,500 to $5,000 annually.

After patients are first screened, a social worker assigned to the pediatric cardiovascular unit works with families to learn about their financial situation. If financial hardship is determined, the family is referred to DOCF for aid. Our program fully covers critical material expenses including medication, oxygenators, disposables, pericardium patches, conduits, valves, and blood product through $5,000-per-surgery contributions. This support, in tandem with the hospital’s pro bono coverage of any remaining gap, is critical to offering life-saving surgery to these families.

To date, the David Ortiz Children’s Fund has provided life-saving surgeries for over 935 children, and funded 1,158 total heart procedures.
